Supported Accommodation:

At NDIS Sydney, we understand that living independently is at times a difficult achievement but with the support of NDIS Sydney we can help make it a reality.

Our services incorporate assistance to achieve living independently whether you are living in your own home, living at a rental property or living in a shared accommodation arrangement.

– We will support you in managing your living arrangements with others.

– We will help you cope with your responsibilities around the home.

– We will support you with your move whether it be living on your own or into shared accommodation

– We will teach you how to live independently and assist you with basic daily tasks like healthy meal preparation, planning your daily activities and ensure a stress free lifestyle.

Household Tasks:

We offer assistance with domestic chores, including assistance with dishwashing, cleaning, clothes washing, fresh linen and ironing, in order to maintain a healthy, safe and secure environment for our participants.

Our services include all the vital cleaning areas in your home:

These include:

– living areas and bedrooms

– bathrooms

– toilets

– kitchens

– laundries

Our support workers engage in tasks such as cleaning stoves tops, wiping down bench tops, cleaning inside fridges as well as vacuuming or mopping floors, dusting, dishwashing, changing bed linen, washing, drying and doing essential ironing.


Therapeutic Supports:

Therapeutic supports (therapies) are offered through NDIS to advance independent living and functional abilities. They are supports that help with all of the routine, everyday tasks including grooming oneself, moving around, interacting with others, fostering positive connections, and taking part in community events, in addition to gym access.

We can also assist you in connecting with a large network of medical experts and specialists, such as:

– Social workers

– Counselling

– Mental health nurses

– Occupational therapists

– Dieticians

– Exercise physiologists

– Physiotherapists

We can assist you in choosing the therapeutic supports (therapies) from each section of your plan that you can use.

Under the “Improved Health & Wellbeing” support area, NDIS SYDNEY is also able to offer customised Exercise Physiology and Personal Training services to qualifying NDIS participants.

Exercise Physiologists and/or Personal Trainers:

Construct personalised fitness programs.

Assist with exercise in a safe and effective manner.

Design personalised diet and wellness programs

Provide individual goals focused sessions.

Assist with general fitness and strength related goals.

Advise on suitable sporting options, recreation and leisure activities.

Community Nursing Care:

NDIS Sydney provides specialised nursing care to participants with a diverse range of needs, in the comfort of their own home.

We give special attention to those living with a disability living at home. We support the value of independence and empower our participants to sustain and maintain living safely in the comfort of their home.

We will liaise with hospital discharge staff and offer timely monitoring to help patients settle back with support to manage their changed medical circumstances at home.

Our services include but are not limited to:

– Skilled nursing care

– General nursing care

– Palliative care

– Medication administration & management

– Colostomy and Urinary Catheter Care Peg Feeds and Stoma Care

– Wound management

– Liaising with relevant stakeholders (GP’s)

Participate Community and Community Participation:

Increased Social and Community Participation is a funded support category you may find under Capacity Building Supports. This category of funding can be used to pay for art classes, tuition fees, sports coaching and similar activities that build skills and independence.

We help you to meet the goals relating to participating in community, social or recreational activities.

Some activities include:

– Join a sporting club

– See a movie or concert

– Attend personal development courses

– Join a social group

– Take an outing or a holiday

– Visit local library

– Go to community events

NDIS Sydney’s Innovative Community Participation Program is designed to encourage NDIS participants to develop and build on skills to actively participate in their community. We will support participants who wish to be active in the community, make connections and socialise. We will also assist you to broaden your prospects for community participation and employment through one on one practice.

NDIS Sydney provides participants with opportunities to engage within the community and appreciate a variety of recreational and leisure activities and employment opportunities that are suited to each individual’s participants goals, wants and desires. Our program will overall provide you with new opportunities to engage in employment, added independence providing you with the ability to look after yourself and create a network of friends through meeting new people and making new friends.


Support Co-ordination:

Our support coordinator will work with you to formalise and make the best use of the supports in your plan.

We will help you to engage and connect with NDIS providers, community and government agencies to locate a variety of services to help you attain your goals.

We aim to build competence in each individual participant and help you to understand and use your NDIS plan to coordinate your supports.

Home Modification:

When circumstances in your life change so should your home. Home modification may be included in your funding plan if: either access to a room or other parts of the home is restricted if your living and care arrangements have been impacted by the state of your current residence An Occupational Therapist will have to conduct and assess the premises and deemed them in need of home modifications. NDIS participants are eligible to receive home modification services that will help them adapt to new requirements and sustain comforts within their own home. At NDIS Sydney, we are able to help you make the most appropriate choices in hiring the right individuals to carry out work within your home. Our consultants will work with our participants to produce a home modification plan that matches their present lifestyle. All you need to do is tell our consultants what your concerns and aspirations are and we will ensure that we meet your needs as best as possible. Some costs may be covered by the NDIS funding which we will confirm during your consultation.


Assistive Prod-Household Task:

At NDIS SYDNEY we will assist you to live as independently as possible by providing access to assistive products designed to ease daily household activities such as: modified eating utensils, dressing aids, emergency call systems and much more.

In accordance with your needs we will provide you with suitable equipment to reduce the difficulty of household tasks and provide you with more independence. We will help you to attain the necessary confidence to undertake daily tasks all on your own.

Some of the specially designed equipment includes:

Alarms, door bells, adapted clocks, computer aids etc.

Development – Life skills / Capacity building

At NDIS Sydney we focus on development of life skills through training and development to help you live as independently as possible. This includes to help the participant work on goals to develop freedom, confidence and essential life skills that will help you to enjoy life to its maximum possibility.

We teach you to tackle life’s challenges through disability skill development. We will help you attain and grow the necessary skills of communication, problem solving, social and critical thinking.

If you have Capacity Building funding for “Increased Social and Community Participation Activities” in your NDIS plan then we can provide you with access to this support.


Assist –Travel Transport

NDIS Sydney provides support with travel and transport services to enable our participants the ability to travel independently.

We provide travel training to use public transport to enable our participants to develop independence to travel alone as well as providing assistive transport related aids and equipment.

We provide transport support including the reasonable and necessary expenses for private transport including taxis and UBER for eligible participants who struggle to travel independently. This may also include travel to and from school.


Assist – Personal activities

Our staff at NDIS Sydney will make life comfortable by providing our participants with assistance of their personal needs. Our staff are well skilled and friendly and will support you in everyday living tasks.

We will help you with personal hygiene activities such as showering, dressing and oral hygiene. We will also assist you to make your meals in the comfort of your home or entirely prepare everyday meals and help with eating and drinking. We support you with mobility in and around your home. Assisting you to get in and out of bed, toileting and general movement from backyard to around the house.

We support you both in and out of your home by ensuring that we accompany you to any regular or monthly appointments with your GP, specialist or dental clinic.


Assist – Life stage, Transition

At NDIS Sydney, we understand that the transition phases throughout your lifetime can be quite challenging. Some examples include: birth of a child, starting school, moving houses, getting your first job or retiring from work. We will strive to help you manage these different stages in your life by assessing your abilities, needs and goals.

We provide assistance and support for either the short term or long term and aim to strengthen our participant’s ability to co-ordinate their supports and capability to live at home and contribute within the community.

Some of the supports we can provide include:

– Assistance with housing and tenancy

– Support co-ordination

– Life transition development, including Individualised skill development, mentoring, self -care, living skills and peer support

– Assistance with financial planning and budgeting

– Linking you to supportive community services and programs.

Assist – Personal Activities high

NDIS Sydney staff will make life comfortable by providing our participants with assistance of their personal needs especially those who require high level of support. All NDIS plans identify the level of assistance a participant requires.

A participant with complex needs will be provided with a support worker with additional qualifications and an extensive amount of experience relevant to the participant’s complex needs. Our goal is to enable our participants to live as independently and freely as possible. We will provide support within your own

home and in a shared accommodation atmosphere. We will also provide the care you need whilst undertaking any activities such as social, recreational educational as well as employment opportunities.


Assist Prod-Pers Care Safety

NDIS Sydney will help you find the right, safe equipment for the use of personal care of participants who suffer from disability and have special needs.

NDIS Sydney will ensure that the equipment and assistive products are safe and secure for use in your residence. They also will include home modification products that will provide a secured place of occupancy. Some of the equipment we can help you find include personal healthcare products such as

grooming aids, high low beds, pressure mattresses, consumables and continence products, toilets and bathroom equipment, safety equipment.


Assist access / Maintain Employment

NDIS Sydney’s aim is to help create a better life for our participants. We will provide workplace assistance to enable a person to successfully obtain and or retain employment. Our support services include helping you attain your employment goals which will allow us to work together to reach and achieve your employment goals.

– We assist you in developing a career plan to explore and discover what options you have available

– Help you to build on key foundation skills for working in a specific industry

– Help you manage complex barriers to gaining or maintaining employment

– We support you to gain employment through Australian disability enterprise (ADE) (which is a not-for-profit organisation that employs people with disabilities).

– We support you to make the transition from an Australian disability enterprise (ADE) to open employment .


Accommodation / Tenancy

At NDIS Sydney we aim to assist our participants in applying for appropriate rental accommodation.

We help our participants to apply for suitable rental accommodation ensuring you obtain and retain suitable and a safe place of accommodation.

We will provide you with one on one support to assist in:

– Assessment for tenancy

– Practical personalised advice

– Achieving your individual accommodation and tenancy goals

– Liaising with service providers

– Help you overcome any barriers along the way

– Help to apply or undertake new tenancy obligations

– To discover and acquire and maintain alternative housing.

Interpret/Translate:

Interpret/Translate NDIS are language interpreting services delivered by TIS National. The registered interpreting service provider will assist participants with English language difficulty. The translation and interpreter services covers 160 languages and will help to provide clear communication pathways and help you feel more connected with the community.


Early childhood support

NDIS Sydney provides support to help children (0-6) boost and reach their intellectual and physical potential. Support provided early will give your child the best chance of attaining their full potential.

NDIS SYDNEY will be your first point of contact. We encourage and assist families who wish to better understand their child’s support needs and help identify and ascertain the most appropriate response to address those needs.

As a NDIS early childhood affiliate, NDIS SYDNEY works intently with families and carers of children aged 0-6 years to enhance children’s abilities and encourage their participation in daily activities to build motivation and drive for their vision for success.


Group / Centre Activities:

At NDIS SYDNEY, we help participants to engage in centre based group activities. Centre based activities, social and recreational activities promote social connection and friendships with other participants. Our programs facilitate constructive learning opportunities specifically designed for building life skill development at the same time supporting participants to seek their personal interests.

Our personalised activities include:

Life skills development – using a computer, gardening, tying shoe laces, driving a car, making the bed, money handling.

Improving health and wellbeing workshops – communication, organisation, problem solving, self-confidence, adaptability, work ethic.

Literacy and Numeracy programs- our programs aim to improve reading and writing skills and developing basic literacy skills and numeracy.

Recreational activities- walking, dancing, meditation, reading, swimming and playing games.

Community Access – see a movie or concert, join a sporting club, visit local library.
